Go to the health facility

  1. If you have an ailment within the scope of the coverage, go to the health institution.
  2. If you choose one of our contracted health institutions, you can benefit from discount opportunities and your provisioning procedures will be followed by this institution.
  3. In First Health (Complementary Health) Insurance, it is obligatory to use hospitals and institutions with SSI agreements, which our company has agreed upon. Apart from this, transactions made in other hospitals and institutions cannot be approved and reimbursement procedure does not apply.
  4. If you prefer a non-contracted healthcare provider, the provisioning process will not be activated. Submit all necessary documents to Sompo Insurance Inc. Health Service.
  5. The documents that you send to our company with a reimbursement request are reviewed by experts. Reimbursement is made to the insured bank account for health expenses which are eligible healthcare expenses for the policy. (Not valid for First Health Policies)
  6. After the examinations are completed, the payment is made to the bank account number of the insured.

What you need to know about Claims Notification

  1. Notify us of your illness (for diseases covered by your policy) within 15 days of knowing.
  2. After making a claim notification, please send us the documents listed below in order, complete and in original.
  3. Your documents are reviewed by the doctors in our company.
  4. After the examinations are completed, the payment is made to the bank account number of the insured.

Required information and documents

  1. Copy of identity card
  2. The doctor's report and the results of the examination that form the basis of the diagnosis
  3. Hospitalization - discharge epicrisis
  4. If necessary, samples, tape recordings, medical inspection themself and medical inspection results that have been made.
  5. Medical Examination Form filled by the insured's doctor
  6. All procedures and medical examination form, reports and results to be performed by the hospital or specialist doctor,
  7. Any medical information and examination required during the claim process, even if not specified herein.
  8. Note: The above documents are standard and extra documents may be requested depending on the differencies of the claim.
